We understand that urgent situations require prompt attention, and Dr. Michaels M.D. is committed to seeing patients with acute illnesses as soon as possible.

We kindly request a 24-hour notice if you need to cancel your appointment at Urgent Care. Failure to provide this notice will result in a $50 charge. We appreciate your understanding as this policy allows us to better serve all our patients.

For various form completions, we charge a modest fee at Urgent Care. Typically, we can complete these forms within 3-5 business days. It’s important to note that there are no fees for back-to-work/school notes and one-page forms completed during the course of your office visit.

Payment Policy at Urgent Care St. Augustine

We welcome patients with insurance at Urgent Care. Our office contracts with many insurance companies and plans. To ensure a smooth process, please provide proof of insurance for all services rendered. If your insurance information is not presented, or if your declared PCP is not affiliated with FM Medical, your account will be considered a cash account, and we will expect full payment at the time of service.

If Urgent Care is contracted as family medicine preferred providers with your health plan, we will directly bill your insurance company. However, if we are not contracted providers with your insurance company, we request payment in full at the time of service. Our team will be happy to assist you in obtaining the necessary information to submit a claim for eligible reimbursement from your insurance company.
